Job Summary And Qualifications

Under the general direction of the Senior Vice President of the New England Heart Institute and Physician Practice Network and the direct supervision of the Department Director of Cardiovascular Wellness and in accordance with the nursing practice standards established by the Nursing Organization and, additionally, by the specific unit/department in accordance with the rules, regulations and/or standards established by the State Board of Nursing, the Joint Commission, and appropriate specialty organizations relevant to the patient population under care, the Cardiac Rehabilitation Nurse Specialist performs the following functions:

  • Demonstrates proficiency in initial and ongoing physical and psychosocial assessment by:
  • Interviewing and examining
  • Participating in patient care conferences
  • Collaborating with physicians and other interdisciplinary team members
  • Obtains and documents a comprehensive cardiovascular history.
  • Assesses behavior and developmental patterns of the patient/family unit from a holistic perspective.
  • Obtains clinical data from diagnostic tests on a continuous basis and incorporates in the assessment process.
  • Uses and combines subjective and objective data to anticipate potential crisis situations and intervenes appropriately.
  • Communicates patient status and assessed needs to other members of the health care team in a professional, organized and timely manner.
  • Collaborates with all members of the Cardiovascular Wellness staff to develop an individual plan of care by utilizing the nursing process and accepted nursing diagnosis.
  • Establishes expected clinical outcomes of patient care based on an interdisciplinary planning approach.
  • Collaborates with the interdisciplinary team to coordinate discharge planning and make appropriate referrals to community agencies.
  • Participates in formulating clinical nursing decisions based on advanced knowledge.
  • Collaborates with all members of the Cardiovascular Wellness staff, nursing staff and other health care team members in developing family/patient educational programs to meet the needs of the cardiac population.
  • Works cooperatively with members of the Cardiovascular Wellness staff to obtain appropriate client referrals for the outpatient cardiovascular wellness classes.
  • Implements interventions tailored to the Cardiovascular Wellness population.
  • Documents nursing interventions, patient response, effectiveness, complications, etc. on the appropriate documentation form.
  • Facilitates communication among health care team members.
  • Respects the dignity and confidentiality of the patient, serving as an advocate when necessary, and provides a safe and secure environment for the patient.
  • Uses all available in-house resource personnel and departments for treatment and consultation.

What qualifications you will need:

  • Active NH Registered Nurse license or Compact State Registered Nurse license
  • Associate Degree in Nursing
  • Minimal requirement of 1-2 years of critical care, cardiac, or cardiac rehabilitation nursing.
  • Advanced Cardiac Life Support must be obtained within 30 days of employment start date
  • Basic Cardiac Life Support
